In the event the registration certificate is lost or damaged, the applicant is entitle to request Roszdravnadzor to issue a duplicate of the registration certificate (hereinafter — application for issuing a duplicate).
If the original registration certificate was damaged, the damaged certificate must be submitted with the application.
Within 7 working days upon receiving the documents listed in paragraph 52 of the Rules, Roszdravnadzor shall issue a duplicate of the registration certificate using a registration certificate form marked “duplicate” and “original registration certificate is from now on deemed invalid” and hand such duplicate to the applicant or send it by registered post with advice of delivery.
It should be noted that in order to obtain a duplicate of the registration certificate, the applicant must also submit a document confirming the payment of the state duty equal to RUB 1,500 together with a duly notorized letter of attorney from the medical product manufacturer confirming the applicant’s authority to receive a duplicate of the registration certificate.
